VOLUNTARY ASSUMPTION OF RISK, WAIVER OF LIABILITY AND INDEMNITY (18+)
I know that speed skating is a potentially hazardous activity. I agree not to participate as a skater with the Toronto Speed Skating Inc. (the “Club”) unless I am medically able and properly prepared. I certify that I have no known physical or mental conditions that would affect my ability to safely participate in the skating sessions, events and related activities.
I agree to abide by any decision of the Club, Speed Skating Canada, the Ontario Speed Skating Association, the City of Toronto, Ford Performance Centre and Lakeshore Arena Corporation and any of their officers, directors, coaches, volunteers, employees or other representatives (herein collectively called "Club Organizers") concerning my ability to participate in skating sessions, events and related activities. I assume any and all risks associated with my participation as a skater, including but not limited to, falls, contact with other persons or objects, and rink conditions.
As a condition of my skating with the Club, I, for myself and anyone entitled to act on my behalf, waive and release the Club Organizers from present and future claims and all liabilities of any kind, known or unknown, arising out of my participation in skating sessions, events or related activities organized by the Club Organizers, even though such claim or liability may arise out of negligence or fault on the part of the Club Organizers. I agree that the Club Organizers shall not be liable for any personal injury, death or property loss, and I release the Club Organizers and waive all claims with respect thereto.
I understand that it is my responsibility to continuously monitor my own physical and mental condition during the skating sessions, events and related activities, and I agree to withdraw immediately and to notify Club Organizers if at any point my continued participation would create a risk of danger to myself or to others. In the event of an injury to me that renders me unconscious or incapable of making a medical decision, I authorize appropriate Club Organizers and emergency medical personnel at the skating sessions, events and related activities to make emergency medical decisions on my behalf (including, but not limited to CPR and AED). I authorize Club Organizers to secure emergency medical care or transportation (i.e., EMS) when deemed necessary by Club Organizers.

I certify that my minor child/ward has no known physical or mental conditions that would affect his or her ability to safely participate in the skating sessions, events and related activities.

I understand that it is my responsibility to continuously monitor my minor child/ward’s physical and mental condition during the skating sessions, events and related activities, and I agree to withdraw my child/ward immediately and to notify appropriate personnel if at any point my child/ward’s continued participation would create a risk of danger to him or herself or to others.
